Some need it done and some don't. If scooting, biting, etc., then it is probably time. The ones that go to the groomer and keep getting it done may have more of an issue because they just stop expelling themselves. Also, firm stool should help.
You can empty them internally or externally. There is a lot of talk about it being easy to cause damage, but I have not found that to be the casue. I've been told this isn't true and there is no special method to it. It's not like a vet can squeeze glands better than an owner... Of course check with your vet first though.
