I neutered a one year old and yours is a very VERY valid question. I didn't dare ask, don't know why, so my answer came with time. Right after the neuter there will be loose skin, but as time goes on, it all tightens up down there. Now, you can't even tell on Kaji. He's all "smoothed out."
