I don't know what airling you're using, whether or not he will be in cargo, or what the temps are like there. But a 4 hour layover is a long time no matter how you look at. I'd at least have your friend go and sit with him for part of that layover if possible. I'm sure the airline employees will do their best, but they also have other duties to attend to.
