Im having this problem too.
Poppy was really good on her pads at the beginning but I didnt allow her as much room to play when she was smaller.
When she is alone she is in her pen in my bedroom, I have wood floor in there and she never has an accident and always keeps it on the pad.
But I think when we are playing downstairs where there is carpet maybe she finds it harder to tell what is pad and what is not.
Im going to start putting the pad in the kitchen where there is wood floor, hope this helps
