Who told you you need a health certificate? If he is flying in the cabin with you, you don't need any paperwork, though I recommend keeping a copy of shot records with you just in case. If a health certificate is needed, it has to be dated within 10 days of your departure from your vet.
The bag you have sounds just fine. I would recommend having a lightweight blanket in case it is cold on the plane. Also have pee pads on hand - they are great for using in airport bathrooms. Make sure he has ID on him during your travels. Most airports will not let a dog on a leash in the airport, but a leash is definitely recommended as well.
