I agree. There are no accidents. The dogs are just doing what comes naturally in that situation ... it is the humans that didn't do what was necessary to prevent the pregnancy. If you fail to plan, you plan to fail.
I also suspect that some of these "accidents" weren't really unplanned. The stories always seem to be inundated with a plethora of excuses as to why it happened giving the OP absolutely no responsibility or control over the matter ... usually the dog was left in the care of someone else, etc. Hey, it's YOUR dog, get it spayed/neutered, and don't make it a baby sitter's responsibility because you didn't take care of business. You would have leave your pet in someone else's care for what, 2 or 3 weeks?, for it not to be in heat when you left, only to have it suddenly go into heat while you are still gone and reach the fertile days.
