Do you mean spiral like it's growing into like a macaroni shape? I've seen this happen to a customer's mini poodle. She didn't take too good care of her, and her nails were super long. Well one of the dewclaws had almost grown into a complete circle. Sometimes they can grow spiral back into the skin. If I'm understanding you correctly, the nail needs to be cut, but if it's that long, you will have it cut bit by bit, since the quick would have grown out long as well.
