When I called they simply said to pay for him when you check in. We weren't able to check in at the self check in counter, we had to get in line with the people who were going to check in their luggage. (we didn't have any) They didn't give me any forms or any confirmation that his "seat" was booked. Once I paid for him the only thing I had as proof was the receipt. I wish they would give a pet boarding pass or something because the receipt simply said, "$200 cash."
