No I did not use the pet passport forms from the website
His "passport" was not really a "passport" per se.... it was just a grouping of papers that addressed each of the items I outlined above. I put everything into one folder in my carry on luggage.
My vet gave me one form - that had Remy's information, his signature. It was a USDA form. That was it. I just added that to my stack of papers...
