You need to watch her when she allowed out of her pen/crate. Not just keep an eye on her when your doing other things, but really watch her like a hawk. If you don't correct her when she's in the act of pottying in the wrong place, she will learn not to do it. If you try to correct her after the fact, then she won't understand. Also, if you clean your carpets with an enzyme cleaner to eliminate all traces of urine, that would help her out too.
