It might be time to go back to basics. If her accidents are as frequent as you described, she might not be ready for all the freedom. Go back to crate training, or putting her in her ex-pen (whichever you used) and limit her freedom unless you're able to watch her like a hawk. Also, treat your carpet with Nature's Miracle if you haven't already. This stuff has enzymes to break down any residual pee that is in your carpet. Just because other cleaners clean to your satisfaction, doesn't mean anything is left behind for your pup to smell. Any left over smell is an open invitation for your pup to pee on your carpet.
