Did your puppy just recently get a vaccination if so how long ago?
This is neurological so get your pup to neurologist if vet cannot figure this out from blood work run for infectious disease and checking liver enzymes. If nothing shows up on blood work get your dog to a neurologist NOW. If it were me i would just go straight to neurologist as they can do all the testing there for everything and do a full neurological exam before any testing.
