Sad Yet another Potty Training Topic

Hello all! I'm a proud mommy to a 4.5 month old Yorkie named Taco. I've had him for over a month now and since the first week, he's been awesome - I'd say he's had a 90% success rate on peeing on his puppy pads. I am indoor training him (we don't have a yard or access to one, living in a city on the 15th floor) and honestly, it's been great.

Then we got him neutered.

Ever since (for the last week) he's been peeing randomly around the house - not spraying but just squatting and peeing. His 90% has dropped to about 50%. It seems that he'll go on the pad when I'm watching him, but when I'm not, he likes to test his limits. He sleeps in his crate so I know that he can hold it for about 5 hours.

How can I get him to go back to his 90% success rate? I'm home with him often but he usually just runs around the apartment (it's not all that big).
