Something is not right, so lets try to figure this out and get your little one to "get it".
What size crate do you have her in?
It should only have enough room for her to turn around and lay down. She should also not be kept in the crate for longer then 20 minutes if she has not peed.
How often are you taking her out and for how long?
When you are crate training she needs to be taken outside for potty every 20 minutes. Pick her up and put her on the ground and tell her "go potty". If she doesn't go within 5 minutes take her back in the house and either into the crate or tethers to your side at all times. If she potties give immediate treats and praise. If she doesn't potty at your side or in the crate.
Do not let her have free rein of your house until she is fully trained. At 16 weeks she is way too young to be any where near fully house trained. You need to be consistent with training or she will never learn.
