Agree! Anytime there is blood in urine, they ARE in pain! My foster Katie was neglected by her previous owner when she had blood in her pee and it developed into four HUGE bladderstones! This was in the span of over a year, and your pup has had it since he was 3 months and is now 3 years? You need to find another vet!

This is a pic of her stones. See how huge they were? NOT good.
