Interesting. I've only paper trained one dog, Jillie, and I haven't run into that problem. I have one pee pad in the house, and she uses it when she has to.
Do you have the pad in different locations around the house? That's the only thing I can think of. When I gave her the freedom of the house, I placed the pee pad in one spot, and I keep it there all the time.
I haven't tried using a pee pad in someone else's home, so perhaps I'll run into that problem too.
As for what to do if she goes where she's not supposed to, if you catch her in the act, give her a "no: and carry her to the place you want her to go, either outside or to a pee pad. I wouldn't scold or punish beyond that. I've heard that when you make the punishment too severe, they just start trying to hide it from you.
