We sign general consents to treat, but not specific to vaccines.
The only time I've signed other consents are when procedures are done that carry risks - spay/neuter, etc.
Also as for the puppy shots. Teddy had his first shots from his breeder. He was done with all his shots, including rabies by 16 weeks old. He also had bordatella as have all my dogs.
