Mike, I dont know the answer to that either. I would almost leave it blank. OKYR did provide a poster that a PET PI did and I have sent that to Shana. It is a GREAT poster.
I am SO mad about Ft Worth. Im trying to get my words together so I can send them a strongly worded email to their "Pets are Worthy" program. Really? Are they? Because I think an HOUR wait and then they wont take any information. Grrr!
