They should be expressed if they need to be, some dogs need them done frequently, some never need them done... so there is no set schedule.

Certain foods may just not work for certain dogs, even though they are high quality... I had my dogs on FROMM and they never had any issues, never had their glands expressed... I switched them to California Natural and they started having a major problem needing them expressed, so I switched again to Innova and now we dont appear to be having any more problems.

You can try adding fiber to your dogs diet (like canned pumpkin), other suggest adding green beans to try and bulk up the stool.
