I have VPI and have used it only for the routine puppy stuff. They have been fairly quick in paying their part (about 2 weeks after submitting) and the forms are simple. You can download them from their website if you dont have them. Just have your vet fill out the top portion and sign it. Send it along with your detailed receipt from the vet. You can fax the form or mail it to them. I hope this helps a little.
