The dog's name, my address and home telephone are on Molly's tag. But then again, we aren't planning on moving anytime soon and have already lived here 13 years.
In your case, the dog's name, cell number and e-mail as another suggested would be my suggestion for you.
I'd never thought of using e-mail on the dog's id tag before, but that's a very good idea as most people have both access to and use computers.
