I think it sounds like she understands the special needs of small dogs during surgery. If you don't take care of the teeth now, you will have to address it in the future. I rely so much on my gut feelings with people/doctors - I think you need to decide how you feel about this particular doctor.
Another thought - do you brush his teeth regularly? If so, maybe it would be ok to wait until his first cleaning and have these teeth pulled then...that would save one time under anesthesia.
I'm sorry, I'm just talking off the top of my head. Usually, I recommend getting the retained teeth pulled as early as possible but if his are in good shape I'd really think about waiting for that first cleaning.
