Poor baby!
I would have your vet check her legs and seriously talk about the anxiety. Sometimes they don't let you know they are in pain. The hyperness might be because of it too. She might be scared when you leave her because perhaps she is in pain and her extremely hard playing could be aggression because of pain too.
