Sentinel is another possibility. I've used it on four toy dogs with no side affects. They have a dosage for 2-10 lbs. One tiny pill; I put it in a little peanut butter and they love their "once a month treat."
It prevents heartworm, whipworm, roundworm, hookworm. It also controls fleas by not allowing eggs to hatch. I haven't seen a flea since I got my first Yorkie in 1997 (he was infested when I got him).
