Are you feeding her prior to the heartworm med. Giving on an empty stomach can cause the reaction you're getting.
You haven't mentioned what brand you are getting. Intercepter is packaged closer to weight and does have a puppy strength....you may want to discuss this with your vet.
I see you live in Oklahoma.....mosquitoes are rampid there....
