This is an excellent article and encapsulates all the important information, in a nutshell.

I only want to add that you must create an environment where your puppy will be sucessful. Do not allow for failures! Most of the time that means containment in small areas and eagle eyes on the puppy all the time! this gets tiresome and burdensome, but in a few short months you will have a housebroken pet. It will be worth the few months of diligence for the many years of accident free home.
