Any regular practice vet should be able to handle the treatment of renal failure. I would make an appointment with another vet for a second opinion since time is off the essence (it does not sound like this is something that can wait) .... internal med specialist is good, but honestly I don't know how a regular vet could not handle this. Of course that all depends on how severe this is. That temperature is worrisome. I am wondering what else is going on.

Too many things we don't know here.
