Whether your goal is to continue using pads or go outside, either way you will have to rid the accident spots of the urine scent. As Guliverandmaya wrote, the scent is attracting your dog to return to those spots.

Some people advocate using an enzyme cleaner like Nature's Miracle, other people have had success with a white vinegar/hot water solution.

Once you have the carpet cleaned, keep a close eye on your dog and try to move her to the pad if you see her miss again. It could be that she slightly missed the pad the first time and then the scent just kept spreading wider and wider.
