On the average you can empty glands via the external method. Every once in awhile you have one that is a bit more difficult, so you need to go in internally to get them cleaned out. Most vets just automatically go the internal way....but, then I don't take them to the vet to have them expressed unless I'm not having success. If one isn't instructed on how to do this you can cause injury and have the anal glands burst on you.
