Hi there, How old are our babies? If they are young and still going to be growing I would suggest a medium so this way you can sill use them later on. The more important measurement is the "adam's apple to tip of nose measurement" as this is what keeps them from reaching around.
Hot Rod is just at 5 lbs and the small works well for him but he is pretty much done growing at 2 1/2. His "adam's apple to nose" measurement is 4" and his neck measures 6 1/2".
