It sounds like the hair is stuck, he shouldn't be having an erection all the time, and if the penis is actually out, that can be very painful. I've read about using ky jelly to slip it back in. The pee string, doesn't really have to be that thick, I trim the hair around the penis, and the string itself is pretty thin.
