Default head hurts and urine

My little boy has had a rough month. Currently, he has a urination problem that the vet can't seem to diagnose. He is on the SO food formula. What he is doing now is going to his pee pad over and over. He stands there and makes funny noises as he shakes his back leg trying to pee. He will leave, and then run back and do the same thing in about 5 min. Sometimes, he will do this over and over. The vet has done xray, urinalysis, and ultrasound. No stones were found anywhere and his urine is clean. He is on clavimox, but that does not seem to be helping.

Earlier in the month, he smacked his head and was totally unconscious for almost 20 minutes, We thought we was dead with a broken neck. When I came home from work, in his excitement he was spinning round and round and hit is head on the wall. He dropped immediately, no staggering, just down like a dead dog who was shot. His eyes were open and his tongue hanging out. We rushed him to the dog hospital and he starting coming around in the car. They kept him over night. He is perfectly fine now. We think he sticks his tongue out more than he used to. His pee problem started the day we brought him back from the hospital.

What is the difficulty diagnosis this kind of urination problem? He seems to be getting the urine out ok.
