In the past, my vet wouldn't prescribe heartworm meds without a test. For years now, I've given heartworm meds 12 months a year, and purchased them from Australia...they don't require a prescription. All has been well for 3 yorkies. I may get a test for Zoey this summer, since I just got her last March...but that will be it.
