We the undersigned, acknowledge and agree to the following terms and conditions in purchasing a Yorkshire Terrier from (Breeder Name). To the best of my knowledge and belief, this puppy (dog) is in good sound health, free from symptoms of contagious and infectious diseases at the time of sale.

(Breeder Name) guarantees the health of the animal at the time of purchase, accepting no responsibility for illness or death brought on by improper care, handling, or accidents once the animal leaves these premises. The animal has had all necessary vaccinations and wormings, with the exception of rabies. A required visit to a veterinarian should be made within a 48 hour period after purchase to insure the health and soundness of the animal. If the animal is not in good health, due to disease only, all money will be refunded in full or a replacement puppy given (the buyers choice) providing a vet's certificate is provided to the seller. This guarantee does not include patella luxation, occurring sporadically among toy breeds and sometimes caused by trauma (note: patella problems have not been detected in (Breeder Name) yorkies nor the presence of worms which may be found in any healthy puppy. No treatment is authorized. This guarantee is made for the protection of both the buyer and the seller.

(Breeder Name) agrees to be available for advice in both the health and training of your new Yorkshire Terrier and reserves the right to have first choice of obtaining the dog should the owner agree to give the animal up, under certain circumstances. It is further understood and agreed that no warranty or representation has been made with respect to the purchased animal, except as set forth in writing in this agreement. All parties will acknowledge they have read and fully understand the terms and conditions of this sale by signing below.
