Default Collar Dangers

My heart just sank! I am in the process of trying to get my 16 week old puppy used to a collar so she can wear her rabies tag. I thought I had it fairly tight. I walk out into my living room to find her with her collar in her mouth, unable to get it out and in a panic. Collars scare me just for this reason. Too tight is not good, too loose is not good. What rules do you go by in getting them used to a collar. I am always scared they are going to get hung up on something in my house and strangle themselves, or get it in their mouth just like she did.
