I would stop the activia as some dogs are lactose intolerant and it can cause them to get diarhea and fortiflora is a probiotic but it needs to be given 2 hrs after the antibiotic. I would mix in the white rice to normal kibble to firm the pup up as long as you give with the kibble the stomach will be ok it is when you switch cold turkey that it can be upsetting. Did you just recently switch to this food? Salmon can be rich is there any reason you are feeding salmon? Sweet potato is ok just no white potato but dogs with colitis and ibd do better on grains than potato i have found. so a diet with rice may be better but NOT BROWN RICE
