Report it to Animal Control. They won't kill the dog, but they may lock it up and the owner will have to pay a fine to get it back. Maybe that will be enough to make them keep their dog under control.
The dog has already gotten loose several times, and you need to establish a formal record with Animal Control to start documenting the problem. It is only a matter of time before the dog hurts another animal or a human, possibly even a child. You do not want that on your conscience, thinking you might have stopped it if only you had made a report earlier.
