Generally that type of ear problem is caused by food allergies. All the medications in the world won't help if it is. My bet is she's feeding some kind of kibble. I would take the dog off all kibble and ALL commercial treats, and then put on brown rice, sweet potatoes and red meat, and if that clears things up in a couple weeks or month, then just keep with the home cooked diet, but only add one new food for a few days at a time. This way she will know if there are any "real" food allergies.
