well, if she has baby teeth that are STILL In at 18 months- she needs to have them pulled- no question. That is definitely leading to her poor teeth condition. So that would be a good time to have them cleaned. Some people do them yearly, every other year. Once she has had them done- you can avoid having them done again by brushing her teeth daily with special dog toothpaste and a toddler toothbrush. I brush Layla's daily- she is going to be 4 and her vet is always complementing me on her teeth.
