Good tips. I have a couple more that might help.
I would get my camera out all the time, but ignore your baby. Flash it, etc. Maybe she'll get used to it, and figure it doesn't have anything to do with her.
Back up. Maybe she won't notice the camera as much. Use a zoom if you have it, or crop it digitally to make her the focus of the pic.
Hope you have digital! You only need a few "keepers"!
