Bald spots can indicate you are putting the hair up too tight. Joey's breeder said that you should slide a comb between band and scalp to make sure that the band isn't too close to scalp. She said that she has seen Yorkies, whose band has been so tight that a black spot develops, which means the skin is dead, so no hair will ever grown in the spot again. Thyroid problems can mean sparse coarse hair, but if the spot is circular, and where the topknot is, I would guess the band is too tight.
