Not much to it, I bought a spider prop similar to this,
Amazon.com: Large Hairy Poseable Black Spider ~... and hot glued velcro to the bottom. I sewed another piece of velcro to the back of a black hoodie, and his stockings are cut from stretchy black knee-hi nylons and connected at the back of his neck. I'm not sure you'd want to take it to nursing home, some angles look pretty realistic, and you might give someone a heart attack!
