It sounds like he may have been given a shot in that site and developed vasculitis there...rabies is often the most damaging to the skin integrity when vasculitis happnens. They are supposed to note the location whenever a shot is given, so it'd be important to know which shot caused this reaction, for the future -- bc it could happen again, and even worse next time.
