This is quite common as someone posted and is a good reason to avoid the bone shaped tags in particular and any dangling tags at all. I leave a break\away cat collar with a flat ID tag attaced flat on the collar, not dangling to my yorkies at all times. They also make bendable rubber tags so they collapse if they get into anything and pull on them.

When we leave the house they wear harnesses too. Mine are microchipped as well since I want every possible ID in case they did get lost or stolen.

I am so glad you were home and Bandit was okay.
