Before going to a shot clinic, I would make sure that there is a vet there doing exams. This is the most important part of the yearly visit. I wouldn't vaccinate before an exam.
Vaccine doses are generally the same for all dogs. That's because it takes a certain amount to get an immune response, so the size of dog doesn't matter.
Rabies should be given separately (by at least two weeks).
You could get the DAPP and bordatella together. I'm not saying it's best, but it should be okay. It looked like corona was in that combo and you may want to avoid it.
