it is because your vet is not doing antibiotics long enough is why it came back. They have to be on it for 21-25 days - what antibiotic are you using? If it re-occurs after 21-25 days you need to do a skin culture sent to lab to determine what antibiotic needs to be used - we just did this as we go to a dermatologist as my dd has allergies. Have the vet give you simplicef for 21-25 days and use chlorhexedine shampoo as it is anti-bacterial and you can get the duoxo brand.
Some have had success with doing chamomille tea rinses as well if you want to go more natural
