a lot of the time if they scrape just one spot they won't find anything. so taking a scrape of multiple spots where there is hair loss will get you better results. A culture like Ann mentioned is also a great idea to see if anything is growing.
She brought up ringworm. did your vet turn out the lights and shine a black light over the hair loss to test for ringworm? while its not a 100% accurate it does show some ringworm.
I see your feeding puppy food so i'm assuming she's rather young. hair loss on a young puppy can sometimes be demodex mange but that should have been caught on the skin scrape.
As far as the food, i personally do not like eukanuba/iams corporation so i do not recommend feeding any food made by them. i don't know which one your feeding but the eukanuba small breed puppy has by products and corn meal 2 of the first 3 ingredients which are not things you want to be feeding your dog. by-products are basically anything left over (feet, beaks, toes, tails, feathers...among other gross things). I'd switch to a higher quality puppy food. many here feed innova, merricks, wellness, or solid gold, just to name a few.
