It sounds as though u r really discouraged & it is very frustrating. I remember so well going through that stage, but the only thing that really worked for us was praise, praise, praise. Some really want to stay close by your side, which is why she is constantly watching for you when she's going outside. She is very small, so she could be frightened outside to be far from you.
I also remember wanting my girl to be able to play and have room to run around in the house, but we had to go back to step one. She would be playing with us with the ball & suddenly stop to go to the bathroom right there. She was just excited about playing & forgot. So back to being secluded in one small area was what worked, but not in a kennel constantly.
Puppy pads worked great for us, but I know u don't want to use them. It really helps to be disciplined about keeping them in their own small space to get use to the puppy pads. Bella never has accidents inside. I thought it might be a good option since u r so discouraged about her going outside. Most who are puppy pad trained also have no problem going outside as well.
