Default Kidney Failure Questions

My 4 year old Yorkie came in contact with leptosperosis almost two years ago and he has never been the same. he was treated for the lepto, but it turns out it caused damage to his kidneys that seems to be worsening at a rapid pace. he has been in and out of the vet multiple times over the last two years and now we have him on a special diet and inject him with fluids on a daily basis. He rarely eats...maybe one meal every two day and he smells horrible. i feel bad complaining about the smell while he is sick, but the odor in my house over the last few days is like nothing I have ever experienced before...its almost like amonia. Also, he can no longer go a night without peeing or pooping on the floor. I am just wondering if anyone has suggestions for me...any thoughts would be greatly appreciated and I am willing to try anything
