Did this start all of a sudden? Has he always been like this?
Finding a great vet is the first step- and can be very hard. I would talk to him about surgery. I think it can be a risky surgery, but it sounds like it can't get worse for this little guy
I would also suggest adding pure canned pumpkin (not the pie filling- 100% pure pumpkin) it does wonders at firming up their poop- it can't hurt. Any firming will help him- see how much of it you can get him to eat. Might take a few day to take affect.
